All electronically-prepared entry forms must be cut/trimmed to individual size from the A4 page, and submitted as three seperate forms.
 

It is the owner/handlers responsibility to ensure that dogs' names appear on all entry forms as on their most recent registration certificate, ie, with complete and up-to-date titles acknowledged.
